Coverage for GLP-1 medications prescribed specifically for weight loss (as opposed to diabetes) remains inconsistent across U.S. payers, and has been a moving target as the cost of covering a large patient population collides with strong demand.

Employer plans

Some large employers added or expanded weight-management drug coverage in response to employee demand, while others scaled back coverage or added stricter prior-authorization and BMI-threshold requirements after seeing costs rise faster than budgeted.

Medicare and Medicaid

Traditional Medicare has historically been barred from covering drugs for weight loss alone, though it can cover GLP-1 drugs prescribed for an on-label indication like type 2 diabetes or, more recently, for reducing cardiovascular risk in patients with obesity and established heart disease. State Medicaid programs vary widely in how they handle GLP-1 weight-management coverage.
