FDA Approves Wegovy HD, a Higher-Dose 7.2 mg Semaglutide Injection

On March 20, 2026, the FDA approved Wegovy HD, a 7.2 mg weekly semaglutide injection — three times the previous maximum Wegovy dose of 2.4 mg — for chronic weight management in adults with obesity already tolerating the standard dose.
Who it's actually for
Wegovy HD isn't a new starting dose. The approved use is for patients who have already tolerated 2.4 mg for at least four weeks and for whom additional weight reduction is clinically indicated — an escalation step, not a replacement for the standard titration schedule.
Trial data
In the STEP UP trial cited in Novo Nordisk's approval announcement, participants on the 7.2 mg dose lost an average of 20.7% of body weight, with roughly one in three losing 25% or more — a larger average effect than the standard dose has shown in prior Wegovy trials.
How it got approved so quickly
Wegovy HD was cleared under the FDA Commissioner's National Priority Voucher pilot program, an accelerated review pathway for products the agency deems aligned with a designated national health priority — worth noting because accelerated review timelines don't reflect the depth of the underlying trial data, only the speed of the regulatory process.
Availability
Novo Nordisk said the higher-dose pen was expected to launch in the U.S. in April 2026. As with other Wegovy dose strengths, insurance coverage and pharmacy availability should be confirmed directly rather than assumed from the approval alone.
